All returned checks are subject to a returned check fee. We also reserve the right to apply late charges to delinquent balances and turn unpaid accounts in your name over to collections services at your cost.
We require insurance information prior to services being provided. Due to the diverse nature of many eye conditions, disorders, and procedures, some of the services we provided are covered by your major medical insurance rather than by routine vision coverage.
Please provide us with both your medical and Vision insurance coverage information even if you believe that you are seeing us for a non-medical reason. We will gladly bill your insurance company for their estimated portion, when possible.
If you believe that your insurance company provides coverage for our service, but we are not on your provider list, payment in full for all services is due at the time of service. In this case we will provide you with an itemized receipt and we will be happy to help you fill out claim forms to expedite timely reimbursement of funds directly to you.
Due to the complexity of insurance plans and reimbursement formulations set by many insurance companies, we will do our best to make accurate estimate of your costs for our services. Please remember that these are just estimates and that you will be billed for any other amounts deemed to be your responsibility by your insurance company.
If we are providers for your insurance plan but are unable to verify your coverage and eligibility prior to your appointment, which is often the case for evening, weekend, and holiday appointments, we will still bill your insurance company as a courtesy to you.
However, we will not be able to give you an estimate of you cost share in these cases, and you will ultimately be responsible for all non-covered services, co-pays, co-insurances, and deductibles.
Knowledge of your insurance plan coverage is your responsibility.
